In order to obtain a check, in the amount of $5 for each copy of the record that you are requesting (please, no cash). Please submit your request in writing to: Mashpee Town Clerk's Office, 16 Great Neck Road North, Mashpee, MA 02649. Your request should indicate the name on the record, the date of birth, date of marriage, or the date of death. We would appreciate your indicating a daytime telephone number that we may reach you in case we have any questions.
As you may be aware, all vital records are maintained at the Massachusetts Department of Vital Records.
