Building Permit Application Requirements

Buildable Parcel – The parcel shall have proof submitted and a determination made by the Building Department that the parcel is buildable under zoning.  The proof consists of a deed search for the subject parcel and all adjacent parcels back to 1960 or when the subject parcel was created.  We require copies of the present deed and all previous deeds for each parcel and shall apply to all new construction.

Board of Appeals – There may be any number of reasons that would require an applicant to file with the Board of Appeals.  These are reviewed case by case, and an application will not be accepted or will be returned if mistakenly accepted.

Conservation Commission – Any lot within the jurisdiction of the Conservation Commission will require the approval of the commission.

Application – The application must be filled out completely, be typed or printed, be legible and have the required signatures.  Incomplete applications will not be accepted.

Section:
1.      Fill out completely, with map and parcel numbers.
2.      Proof of ownership (recent recorded deed – per Zoning & 780 CMR State Building Code).
3.      Must have all signatures, telephone numbers, and street address.  Addresses to be confirmed by the Town Clerk.
4.      Must be filled out completely with license numbers, signatures, etc.
5.      The general contractor’s work-comp can cover all trades on the project or each sub and his insurance must be listed on the workman’s compensation affidavit.
6.      A full description of work.
7.      A true estimated values of project.  The value is subject to review and adjustment.
8.      Applications will not be accepted without all required signatures or check-off.  The signature of the Board of Health is required, Conservation, Fire Department and DPW (if applicable) when brought to the Building Department.  A copy of all Board of Appeals variances and/or special permits is required, if applicable.

The follow is required with the application:
1.      A signed copy of the homeowner’s exemption (if applicable).
2.      A copy of the construction supervisor’s license.
3.      A copy of the home improvement registration (work in an existing home).
4.      A completed and signed copy of the workman’s compensation insurance affidavit.  Must include subcontractors if no primary coverage.
5.      A copy of a road-cut permit for all town roads.
6.      A copy of the recorded Board of Appeals special permit or variance.
7.      A copy of the site plan with the setbacks.




8.      An engineer’s site plan with the flood zone and Flood Insurance Rate Map number and date listed.  Flood Zone, ACEC, rivers and all wetlands will be delineated or a statement will be added that the parcel is not within 100 feet of the above or the required setback.
9.      Two sets of building plans – one full size and one reduced to 11 inches by 17 inches minimum.
10.     Energy form or RESCheck – signed and dated.
11.     The plans shall include the following:

a.      Plan view of the foundation, first floor, and second floor.
b.      Views of the four sides.
c.      Cross- section.
d.      All dimensions of the length, width, and height of the house, room size, and openings.
e.      Size of lumber, posts and beams.
f.      Plan view of the stick framing showing each and every joist, rafter, beam and posts with sizes.
g.      Full deck and porch plans.
h.      Fire protection systems located on the plans.  (Smoke detectors and carbon monoxide detectors).
i.      New construction and remodeling with fire alarm systems and sprinkler systems require a narrative (contract or a set of plans) from the fire alarm company and the sprinkler company.  For more information, refer to 780 CMR 903.1.1 Fire protection construction documents.
j.      Pool permits are separate and a fence permit is required with the pool.

12.     A certified copy of the site plan (as-built) is required prior to a foundation inspection.

By determination the Building Official may waive sections pertaining to construction documents, if the scope of work is of a minor nature.
